Moving into a care home after a stroke: questions about rehabilitation and care

This is not medical advice; the hospital team should set the rehabilitation plan. Talk to the medical social worker before discharge, and see discharge and respite. When choosing a home, ask:
- How often do physiotherapists and occupational therapists come, and how many sessions does each resident get a week? (The voucher's standard package includes two a week.)
- Are there lifts, grab rails and accessible bathrooms? Can a wheelchair get into the rooms?
- How does the home prepare food for residents with swallowing difficulties?
- Who escorts residents to follow-up appointments, and what does escort and transport cost?
- If more care is needed later, can the resident move to a nursing or continuum-of-care place in the same home?
